Kitties are very funny in that these behaviors are definitely sending you a message and it's not always easy to figure out. You sort of have to play detective sometimes. Keeping the kitties litterbox clean at all times is sooooo important and if you constantly dump clean litter on top of wet stinky pee litter then the kitty could be trying to tell you he wants to poop into clean litter by removing it into a pile outside the box and then pooping into the pile. Many times people will clean out the poo but not remove the pee'd on litter for too long. You should dump all the litter from the box at least a couple times a week, then wash it with soap and water before adding new litter. This will also keep your kitty from getting UTI's (urinary tract infections)which can make them very sick. Hope that helps you! =)

it is also a good idea to have some sort of rubber-backed carpet pad around the litter box, mine are at least 8 inches larger than the box in each direction. rubber-backed so nothing soaks thru...i had a box that "leaked" and it was not pretty, i got rid of the box, and the new one had a large pad under it...

I've had a couple of cats that prefer to poop on cold hard surfaces: floor or the shower/bathtub. Sheba is the only one who tries to put litter over it. Nothing I do has ever worked with them--I just clean it up. Purebreds are more prone to this.
