Lucy will head straight to the litterbox and pee. Then jump out and onto the carpet and poo. Or just walk to the back of the box in the middle of the day and poo.
What should I do?

Always pick up the poop and put it in the box. Then pick her up and put her in the box, too. If you can use her little paw to bury it, that's even better. She may not know that's the right place to poop.
Make sure her mistakes have been totally cleaned up, using an enzyme cleaner. If she can still smell where the last offering was, she'll use it again.
There's a product call "Cat Attract" that seems to work for a large percentage of people. You sprinkle it in her box and it attracts her to go there.
The other thing you need to look for is: does it appear to hurt her when she poops? Cats associated pain with places, so if she has had a painful poop in the box, she'll try to avoid the pain by avoiding the box. This behaviour is more common with a urinary track infection, but if she has trouble pooping, that might be the answer.
